PLUS 5/6 Registration Form

PLUS 5/6 is our church's Friday afternoon kids club for children in Years 5 and 6 at school. We aim to offer a relaxed, fun and safe space where kids can thrive, build friendships and explore the Christian faith. We meet during the school term from 4:00 - 6:00pm (includes dinner) at a church member’s residence. Once you register your child, we will email further details, including the address.

To best care for the children at PLUS 5/6, please complete the confidential form below. This is an annual registration to ensure our database is up-to-date with correct contact and medical information.

I authorise my child to travel in a leader's car for emergency situations, following the Safe Church Policy.

I permit photos and videos taken of my child to be displayed in church publications, e.g. website, newsletters, brochures.

PLUS 5/6 Agreement

I understand that certain inherent risks and dangers may exist in the activities in which my child will be participating. I acknowledge that while the organisation and its leaders will make every reasonable effort to minimise exposure to known risks, all hazards and dangers associated with these activities cannot be foreseen or may be beyond the control of the organisation and its leaders. In the event of any emergency where my nominated emergency contact person is unavailable:

I accept the use of calling an ambulance by a trained first aid person if in his/her judgement it is necessary.

I responsibility for payment of all expenses associated with such treatment.

Privacy Policy - Pennant Hills Baptist Church

Pennant Hills Baptist Church (PHBC) is committed to the protection of personal information of the PHBC community. PHBC upholds members rights to privacy protection and adheres to the Privacy Act 1988 (Cth) and the Australian Privacy Principles which regulates the way Australian agencies collect, use, disclose, and store personal information.

To uphold privacy obligations, PHBC must obtain written consent for the collection, use, disclosure and storage of your personal information including your name, address, email, phone contacts, and date of birth. This personal information will be recorded in the PHBC online database (Elvanto) as well as in a hard copy church directory (when deemed necessary). Only the PHBC Administration Manager have access to this online database. The church directory is distributed to Senior PHBC members who do not have internet access so that members have means to contact other members of the church. This also impacts on the function of various ministries within the church, and in particular adhering to Safe Church policy.
